MASTER’S PROGRAMME IN CASE/CARE MANAGEMENT IN HOSPITAL AND COMMUNITY SETTINGS

The series of presentations dedicated to the educational offer of the University of Parma Master’s Programmes in Healthcare for the 2026/2027 Academic Year continues, with applications already open. The programme we are presenting is the First-Level Master’s Programme in Case/Care Management in Hospital and Community Settings for Healthcare Professionals, dedicated to developing advanced competencies in the management of clinical care pathways and the coordination of care across different healthcare settings.

In a health and social care system characterised by increasingly complex care needs, the ability to ensure continuity of care and integration between services is essential to the quality of care. The Master’s Programme aims to prepare professionals capable of providing holistic, person-centred care and managing clinical care pathways, recognising the active role of the family and promoting collaboration between hospital and community services through a multidisciplinary approach.

The educational programme provides opportunities to develop clinical, organisational, communication, and methodological competencies, with particular attention to disease prevention, care management, and rehabilitation processes. Its main objectives include developing tools to improve the effectiveness and efficiency of care pathways, strengthening interpersonal skills in relationships with patients, caregivers, and members of the multidisciplinary team, and acquiring research competencies that support evidence-based clinical practice.

Particular emphasis is placed on continuity of care between hospital and community settings, explored through the study of diagnostic, therapeutic, care, and rehabilitation pathways for people of different age groups, including those living with chronic conditions, disabilities, or frailty. The Master’s Programme also explores healthcare service organisational models, interprofessional collaboration, and the tools needed to design and manage integrated care pathways, with particular attention to person- and family-centred care.

The educational programme combines interactive theoretical lessons, workshops, seminars, and group work, alongside clinical placements and the development of a project work. The Master’s Programme has a duration of one year and awards 60 ECTS credits. Classes are delivered online in synchronous mode, two days a week on alternate weeks, with the exception of July and August. Applications for the 2026/2027 Academic Year are already open and may be submitted until 12:00 p.m. on 29 January 2027.

Choosing the Master’s Programme in Case/Care Management in Hospital and Community Settings for Healthcare Professionals means undertaking an advanced educational pathway that integrates scientific knowledge, organisational competencies, interpersonal skills, and practical experience, with the aim of preparing professionals capable of contributing to the management of coordinated care pathways, continuity of care, and integration between healthcare and health and social care services.
